At Whipple Creek Wellness, we maintain a confidential record of the mental health services we provide. As a patient, you have the legal right to review, request copies, or ask for corrections to your medical record if you believe something is inaccurate.

We do not share your private health information without your written consent, unless required by law.

To request access to your records or ask questions, please contact our office team. Record fees are as follows:

• Copying Fees:
    • $1.00 per page (first 30 pages)
    • $1.50 per page (each additional page)

• Search Fee: $30.00 per request

• Editing (as required by law): Standard office visit fee
